Register as a New Patient

Please visit the reception in person if you would like to register as a patient with us. 

The doctors welcome new patients who live within our practice area.  As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a registration form and health questionnaire and are offered to make a registration appointment with the health care assistant.  As it takes some time for your records to be forwarded to us please ensure you have adequate supplies of medication before registering.  

New patients will also be asked to provide phographic ID and proof of address (i.e. bank statement or utility bill) if you do not have this ID please speak to reception. 

Medical treatment is available from the date of registration.  Please contact reception for further information.

Temporary Registrations

If you are ill while away from home you may be able to temporarily register for 14 or 90 days. After expiry of your temporary registration you must register permenantly or return to your registered doctor. 

Non English Speakers

These fact sheets have been written to explain the role of UK health services, the National Health Service (NHS), to newly-arrived individuals.  They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.

Special care has been taken to ensure that information is given in clear language, and the content and style has been tested with user groups.
